**The Maximum amount of people we can accept for the Banquet is 450. Once the maximum is reached, anyone registering after that will be put on a waiting list for any cancellations. We expect the Banquet to be sold out early, so register and pay as soon as possible. Payment will secure your reservation! Registration/Payment WILL NOT be accepted at the door!

Print a copy of the completed form for your files before clicking on "Check". DO NOT continue on to check-out via credit card. Send your check for the total amount to:
